Maddy ist die Tochter des Regisseurs Stephen Gyllenhaal und der Produzentin und Autorin Naomi Foner. Ihr Bruder ist der Schauspieler Jake Gyllenhaal. Die väterliche Familie ist schwedischer Herkunft und gehört zum alten Adel Schwedens. Ihre mütterliche Familie ist russisch-jüdischer Herkunft.
Sie ist seit 2009 mit dem Schauspieler Peter Sarsgaard verheiratet, den sie 2003 bei den Dreharbeiten zum bisher unveröffentlichten Low-Budget-Film In God’s Hands kennenlernte. Am 3. Oktober 2006 kam die gemeinsame Tochter zur Welt.
